*The membership fee for an additional member (more than 6) will be $150 for each member.
*The membership is for non-insured patients only.
*The membership fee is non-refundable.
*The membership fee is non-transferable.
*The membership is valid for 1 year from the date of enrollment.
*The membership fee is subject to change each year.
